Make a list, in columns, including the following information for each runner. Grouping helps: All girls, by grade; then all boys, by grade. Please arrange your columns as shown below.

In the "Eligibility Column" list the dates of the four meet in which the athlete competed in order to be eligible to compete in the Championship.  It is OK to list a date after the form has to be sent in, so long as the athlete actually competes on that date.

Mail the form you have created in with your check. Do not fax it. Do not e-mail it. Mail it with your check.
Mail the form, with your check, by Certified mail, by October 17, 2003. You may also hand-deliver the form to Thetford Academy before 5:00 p.m. on Monday, October 20, 2003.  Call Dan Grossman before hand-delivering.
