Make a list, in columns, including the following information for each runner. Grouping helps: All girls, by grade; then all boys, by grade. Please arrange your columns as shown below.

Please Note: Artificial 5K times, such as listing all of a team's runners at the same times so that they may run in the same race, makes it very difficult to seed the races. It is our hope that coaches will not use this device. In the event artificial times skew our seeding process too much, we may require runners to run in the race for which they are seeded.

Example for 55 high-school runners: (20 x $4) + (10 x $3) + (20 x $2) + (5 x $1) = $155 total

Mail the form you have created in with your check. Do not fax it. Do not e-mail it. Mail it with your check.

[NOTE: Send an e-mail to dan@dg123.com stating your interest in competing as soon as you know, and then send the forms and money by September 27. Sending the e-mail does NOT obligate you to compete, in case your plans change. It helps us with complying with NFHS sanction requirements.] If postmarked after September 27, include $25 late-registration fee.

Please do all computations of fees for one school on one sheet, even if you have multiple lists of runners.

Entry Fee:

High School -- First 20: $4/ea.; 21st-30th: $3/ea.; 31st-50th: $2/ea.; 51st+: $1/ea.
Junior High -- 1st-20th: $3/ea.; 21st+: $1/ea.
